How we work with these patients
Patients arrive at different stages, usually alongside existing nephrology care. Assessment covers renal function, medications, hydration, and nutritional status.
Assessment covers medical history, current medications, recent laboratory work, and what the patient and family are hoping to achieve. Where a specialist is involved and the patient consents, we write to them.
