Long-term conditions require long-term coordination. Assessment covers history, current management, and what supportive care may be appropriate.
Assessment covers medical history, current medications, recent laboratory work, and what the patient and family are hoping to achieve. Where a specialist is involved and the patient consents, we write to them.

If you’re arranging this for a parent or relative
